Families whose students participate in SIA’s meal programs and require special dietary accommodations should complete the Request for Special Dietary Accommodations form. If you have previously submitted a dietary accommodation request, please contact foodservice@spokaneintlacademy.org to determine whether an updated form is needed.

The Request for Special Dietary Accommodations form must be completed in full and signed by the student’s physician or a licensed healthcare professional authorized to write medical prescriptions in Washington State. Please download the form using the link below.

Spokane International Academy will accommodate the student’s dietary needs while the completed form is pending. To ensure student safety and compliance with program requirements, families are asked to submit the completed medical statement as soon as possible.

Completed forms may be emailed to foodservice@spokaneintlacademy.org or delivered to the Front Office.
