If your student is participating in our meal programs and would like to request a special dietary accommodation, please fill out A Request for Special Dietary Accommodations form (see below).  If you have submitted a special dietary accommodation request in the past, please contact the School Nurse.

A Request for Special Dietary Accommodations form must be completed in its entirety and signed by your student's physician (or a licensed healthcare professional authorized to write medical prescriptions in Washington State).  Please download the form by clicking the box below.  A family needs to return the form within 15 calendar days, during which time the student’s accommodation for special dietary needs will be met by the district.  Spokane International Academy will provide a reasonable modification to keep the child safe, and request the family provide a medical statement signed by a State Recognized Medical Authority to support the meal modification as soon as possible. If your student can not drink Cow's Milk because of lactose intolerance or any other dietary reason and you would like a substitution for School Breakfast & Lunch please submit a written request to the School Nurse.
